Hi Bob,

If you select "Edit / Custom Settings..." from within this report, do you see all of the fields you want listed on the right side, under "Included In Report"? If not, select them from the left, and choose the "Add" button to include them in your report.

If you want to make this same report each year, you might want to memorize this report. After you get it set the way you want, choose "Edit / Memorize...". Then, you can assign it a name, and in the future, this named report will be available under your "Reports" menu.
