"one or more fields not specified" error. I've tried a lot of variations and your wisdom is appreciated.

input format: UD,TRAN,ANUM,BNUM,CNUM

First three rows of data in the .csv file -
UD,TRAN,ANUM,BNUM,CNUM
20051017,BUY,$32.43 ,1.156,0
20051003,BUY,$33.31 ,1.126,0
